Minors: If a minor comes to practice without a parent you must print this form out and your parent must have it notarized. Bring it to the gate when you pay to get it. This is a MUST. Download Minor Release Form (Please note both pages must be signed and notarized). If you do not have your parent or a notarized release you will NOT be able to ride.
